The challenge
Meridian Health operates twelve outpatient clinics across Gauteng. Every booking went through a call centre, every referral travelled as paper, and patients had no way to see their own results. The call centre answered 60% of calls within five minutes on a good day.
The number that hurt most was no-shows. Almost one in four booked appointments went unused, which meant idle clinicians, longer waiting lists, and patients who quietly fell out of treatment. Reminders were manual: a clerk phoning down a printed list when time allowed.
Meridian had a modern clinical records system behind the scenes. The problem was that patients could not touch any of it.
The approach
We led with design research, not screens. Over three weeks we interviewed patients in waiting rooms, listened to call centre recordings, and shadowed reception staff at four clinics. Two findings shaped everything: most calls were for three simple tasks, and the patients most likely to miss appointments were the least likely to install an app. So we built for the browser first, in plain language, in three languages.

What we built
The portal is a mobile-first web application that talks to the clinical records system over an HL7 FHIR integration layer we built alongside it. Patients register with an ID number and a one-time code, then book, reschedule, and view results against live clinic availability. Nothing is a copy: the schedule the patient sees is the schedule reception sees.

The outcome
Six months after launch, 71% of active patients had registered on the portal and the no-show rate had fallen from 24% to 15%, a 38% reduction. The one-tap reschedule mattered as much as the reminder itself: patients who could not attend now free the slot instead of silently missing it, and waiting lists move faster.
Call centre volume for bookings and results dropped by half, so the team now spends its time on complex queries and follow-up care coordination. Patient satisfaction across the clinics averages 4.6 out of 5, and Meridian is rolling the portal into its two new clinics as standard from day one.
